Collection of information
In order to provide appropriate medical care, we will seek a medical history from you or your authorised representative during the consultation. This may involve retrieving past medical records or test results from other medical services or practices relevant to your care.
Storage of personal information
There are no hard copies of personal information kept on site. All information is transfered and stored electronically on a secure system and hard copies are shredded and disposed off.
Disclosure
Your personal information may be shared with:

health professionals involved in your care (GP, other treating physicians or specialists and pharmacists)

Medicare to retrieve information stored in MyHealthRecord
Medical records will not be released to you directly unless you provide a letter signed by yourself advising us to do so for your records or for provision to another healthcare provider.
Data retention
Medical records are legally required to be kept for 7 years even if you are no longer a patient at the clinic.
